provides Home Instruction to students who are absent for ten (10) consecutive school days upon a submitted request from the student's physician and subsequently approval from the district's physician, according to BOE Policy 2481. Parents must download the related Home Instruction Request Form posted below, complete their portion of the form and then forward to their physician for completion.
