Throughout the school year, students may participate in classroom activities, school events, performances, celebrations, field trips, clubs, athletic activities, PTO-sponsored activities, and other educational or community programs where photographs, video recordings, audio recordings, student artwork, projects, or other media may be created.

The school may wish to use this media to celebrate student activities, communicate with families, document school programs, recognize student accomplishments, and share information about our school community.

As the parent or legal guardian of the student identified on this form, you are being asked to indicate whether you authorize the school and its authorized representatives to photograph, record, display, publish, or otherwise use media containing your child in the ways you specifically select below.

Media may include photographs, video, audio or voice recordings, artwork, projects, written work, or other materials created by or featuring your child.

Depending on the permissions you select, authorized media may appear in locations such as the school or district website, PTO Moms school or PTO pages, school newsletters, yearbooks or publications, printed materials, event displays, educational presentations, official school or PTO social-media accounts, or other school-related communications.

Your permission is voluntary. Please review each option below and select the types of use you authorize. Choosing not to authorize a particular use should not prevent your child from participating in ordinary educational activities or school programs, except where photography or recording is an essential part of a specific activity and the school has informed you otherwise.

The school will make reasonable efforts to use student media appropriately and in accordance with the permissions provided. Authorization to publish media does not guarantee that media featuring your child will be used.

Unless separately authorized, this release should not be interpreted as permission to publish sensitive student information, such as a home address, personal telephone number, private contact information, student identification number, academic records, medical information, or other confidential student records.

When identification is permitted, the school should follow its applicable policies regarding whether a student's first name, full name, grade, classroom, activity, or other limited identifying information may accompany the media.

If you have more than one child attending the school, submit a separate authorization for each child unless the school provides another method for recording permissions for multiple students.

Your authorization will remain in effect for the period specified by the school or until it is changed or withdrawn in accordance with school policy. Withdrawal applies to future use where reasonably possible and may not require the school or third parties to retrieve materials that were lawfully published, printed, distributed, or otherwise used before the withdrawal was received.
